We never request or require any personal identifying information about you (such as name, address, email address, etc.) and we do not wish to receive any such identifying information. If you provide us with any personal identifying information incidental to your use of the Site, we will keep it private. When you complete the Questionnaire, we collect the health information (such as symptoms, diagnoses, treatments, etc.) that you provide. We will use the information from the Questionnaire to assist you to organize your health information and generate questions that you may ask of your healthcare provider. We may also use data that has been submitted by you in the Questionnaire, in aggregate form and without personally identifying information, to help improve the Questionnaire, understand who is using it, and use it for marketing or scientific purposes. This information may be accessed by others as necessary for them to perform their duties to us in connection with the Site.
You are fully responsible for all consequences of your use of the Questionnaire. The Questionnaire is provided for educational purposes only. The information that we provide when you complete the Questionnaire is not medical advice and we do not provide diagnoses, prognoses, treatment instructions, or other similar information that should be obtained only from your healthcare provider. You should never disregard or delay seeking medical advice from a qualified healthcare provider because of something you have read, heard, seen or that has been provided to you as a result of your use of the Questionnaire. Although we make a conscientious effort to provide high quality information, we make no guarantees whatever about the accuracy, completeness, timeliness or relevance of any information. The Questionnaire could include typographical errors, inaccuracies or other errors, and unauthorized alterations could be made to the Questionnaire by third parties. In the event that an inaccuracy arises, please inform us so it can be corrected.
You agree that the Questionnaire and the process used by the Owner to provide you with information through the Site are confidential and proprietary materials of Owner. As a result, you agree to keep such questions and process confidential and not to disclose them to any third party and not to use, or induce any third party to use, the questions or the process for development of any products, technology, or services, that refer to or incorporate any part of confidential information in such product, technology or service.

YOUR USE OF THE QUESTIONNAIRE IS ON AN "AS IS" AND "AS PROVIDED" BASIS AND OWNER MAKES NO REPRESENTATION OR WARRANTY WITH RESPECT TO THE CONTENTS OF THE QUESTIONNAIRE, AND SPECIFICALLY DISCLAIMS TO THE FULLEST EXTENT PERMITTED BY LAW ANY AND ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO: IMPLIED WARRANTIES OF MERCHANTABILITY, ACCURACY, TITLE, COMPLETENESS, TIMELINESS, NON-INFRINGEMENT, CORRECTNESS, OR FITNESS FOR ANY PARTICULAR USE, APPLICATION OR PURPOSE. YOU VOLUNTARILY AND UNEQUIVOCALLY WAIVE ANY LIABILITY OF OWNER AND ITS OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS, INCLUDING HEALTH CARE PROFESSIONALS, FOR ANY DAMAGES, CLAIMS, DEMANDS OR CAUSES OF ACTION, DIRECT OR INDIRECT, SPECIAL, INCIDENTAL, CONSEQUENTIAL OR PUNITIVE (COLLECTIVELY, THE "INJURY"), AS A RESULT OF YOUR USE OF THE QUESTIONNAIRE OR ANY INFORMATION YOU OBTAIN AS A RESULT. YOUR SOLE AND EXCLUSIVE REMEDY FOR ANY INJURY IS TO DISCONTINUE USING THE QUESTIONNAIRE.
